b. Official Transcript of Records(TOR)/ True Copy of Grades
Note: If TOR is not available – Certificate of Graduation
For Graduating Students – Certificate of Candidacy for Graduation
c. PSA Birth Certificate
d. 2×2 portrait photo
New students and transferees will also be scheduled for an interview with Dean Israelito P. Torreon.
